Making Appointments | Your First Visit | Insurance Information | Hand Therapy

What to Expect

We look forward to serving you efficiently and effectively during your first visit. Your initial examination at Hand Surgery Specialists will require a thorough evaluation by your physician and may include X-rays or other diagnostic tests.

To design the best and most efficient treatment plan for an upper extremity problem we must study your medical conditions and what tests or treatment you have already undergone. By supplying your medical history as it relates to your specific disorder, you enable your hand specialist to diagnose and treat your disorder effectively and efficiently.

First Visit Checklist

1) FIRST VISIT FORMS: The forms that you will need to complete at your first visit can be found below. Please save yourself time at the first visit by printing these forms now, completing them and bringing them to your first visit.

2) PREVIOUS TESTS OR X-RAYS: We need the results of the studies that relate to your upper extremity disorder.

3) ANY OTHER TESTS: Please ask your previous physician to provide you with copies of tests that relate to your current problem, or information that impacts your overall health.

4) MEDICATIONS: Name and dosages of any medication that you are currently taking.

5) HEALTH INSURANCE CARD: With the insurance company's address.

6) SOCIAL SECURITY CARD OR DRIVER'S LICENSE.

7) YOUR EMPLOYER'S ADDRESS

8) YOUR INSURANCE COPAY

9) INDUSTRIAL CLAIM NUMBER: If your injury is work-related and an industrial claim number has been assigned, please bring this number with you.

10) NAMES AND ADDRESSES OF YOUR OTHER PHYSICIANS: The results of the evaluation will be sent in a report to your family physician and to any other physicians who are involved in your care.

If your medical insurance plan requires a referral from your Primary Care Physician, be sure that the referral is obtained prior to your appointment in our office.

After the evaluation, your physician will discuss your condition with you and outline different treatment options. Empowered with the information necessary to make critical decisions, you and your hand specialist will select a treatment program that is best suited to your individual needs.

We have provided four online forms you should complete before your first visit to our office. Completing these forms prior to your first visit will allow a faster and more efficient registration experience.

If you are unable to print and complete these forms from home, we will provide you with the forms at the office upon registration.

Scheduling Surgery and Tests

If surgery or tests are required, critical steps must be taken. We understand that any kind of surgery can be stressful, and the rules and regulations of insurance companies can be confusing. Hand Surgery Specialists will work with you every step of the way in scheduling surgery and tests. Please make sure that you understand and follow these steps:

· You will need to collaborate with our scheduler to secure an acceptable date for you, your insurance company, your surgeon, and the Hand Surgery Center.

· You must get approval from your insurance company. Ultimately it is your responsibility to make sure that all procedures are covered by your insurance policy.

· You will need to alert Hand Surgery Specialists of rules and regulations specific to your insurance company. Every insurance company is different.

Payment

The fee for the initial examination will vary depending upon the complexity of your problem. Additional charges will be made for diagnostic tests or X-rays and for treatments such as casts, splints, or injections.

Payment for services is due at the time of the visit. All copays will be collected at the time of the service. Payment from the patient is expected if you do not provide the billing office with the appropriate information necessary to submit an insurance claim. Ask your primary care physician to complete a referral if your insurance requires one for a visit to a specialist.

If you are unable to pay on the day you receive services from a Hand Surgery Specialists physician, please discuss the situation with our Business Office so that special arrangements can be made. Since our primary concern is your health, we will make every effort to accommodate any unusual circumstances that make your financial obligations difficult to fulfill.
