Information
Hand Surgery Specialists have designed policies that will make
your experience with us positive and efficient.
Referrals We
understand that insurance and the referral process can be confusing.
Please do not hesitate to ask for our assistance. In order to
accommodate the needs and requests of our patients we have enrolled
in numerous managed care insurance programs. While we are pleased
to be able to provide this service to you, it is extremely difficult
for us to keep track of all the individual requirements of the
plans.

| Your
insurance coverage is a contract directly between you and your
insurance provider. Ultimately, it is your responsibility to
obtain a referral from your Primary Care Physician. Please ask
the staff of your referring physician to make the phone calls
or complete the forms that are required by your insurance provider
to obtain the referral if one is required. |
If
you do not take the responsibility of obtaining a referral, our
facility will have no choice but to bill you directly for services
rendered if insurance payment rejection results from lack of referral.
If
your injury is work related, please bring an industrial claim number
with you if one has been assigned.
With
your cooperation and help, you should be able to receive all the
benefits offered to you, and we will be able to concentrate on caring
for your medical needs.
Worker's
Compensation
Hand Surgery Specialists understands the frustration of getting
injured on the job. We recognize that it may take up to two weeks
to secure a claim number. It is critical that you submit the correct
paperwork as soon as possible. It is your responsibility to file
the claim the day of the injury.
In
the meantime, we can provide services as needed. When you call for
your appointment, please provide us with your employer's name, contact
name, and phone number so that we can call to determine your Managed
Care Provider.
As
a provider of medical services, Hand Surgery Specialists is asked
to provide the following information to state or self-employed providers:
1)
Primary diagnosis
2) Summaries of medical activity
3) Prescribed treatments
4) Prognosis
5) Return-to-work dates
6) Other factors regarding the claims we are treating

Disability Claims
Hand Surgery Specialists is pleased to assist in your disability
claim. A careful review of your medical record and information from
your physician is required for the completion of disability forms.
For this reason, we are unable to offer, "while you wait" service.
Forms will be completed as quickly as possible and in the order
we receive them. Please allow seven to ten working days for the
accurate completion of disability forms. A minimal charge, payable
in advance, is charged for this service.
